Breast Augmentation
There are two types of implants on the market today. Both have a silicone shell, the
difference is what they are filled with.
saline breast implants are filled with salt water.
silicone gel implants are filled with silicone. As you know there has been controversy
over the use of silicone breast implants. They became FDA approved again in late
2006. A silicone breast implant has a more natural feel then a saline implant. If you do
go this route it is extremely important to follow up with breast MRI to exclude rupture.
Both saline and silicone implants can rupture /leak. Saline will be reabsorbed into the
body where as the silicone will not be. The silicone implants made today are more
cohesive then before so if a rupture occurred the silicone should not spill out as it did
before.

Position
Breast implants may be placed above or below the muscle. I prefer to place the
implants under the muscle as there is better soft tissue coverage over the implant
making the implant less noticeable. It will also allow for better mammograms as less
breast tissue is hidden.
Breast Augmentation using fat grafts. This is an option but there is concern about
the amount of scarring post operatively that may prevent malignancy diagnosis. At
this time I am not performing this procedure until the risks have been defined better.
Breast implant with breast lift.
With age and child birth the breast may be sagging and the nipple may be lower on
the breast then it should be. With these patients I must do a combined breast
augmentation and breast lift better known as a mastopexy augmentation.
